    Note that the mucogingival junction is displaced well coronally and there is no attached gingiva

     

    1 horizontal incision, apically located, on the vestibule, at the location to where you plan to have attached gingiva (well beyond the mucogingival junction)

    2 vertical incisions from the horizontal incision, mesial and distal, to the middle of the crest, slightly palatal (or lingual).

     

    2 vertical releasing incisions from the horizontal incision, mesial and distal, to the middle of the crest, slightly palatal (or lingual)

     

    Dissect the flap, split thickness, all the way up to the top of the crest. On top of the crest do full thickness incision and dissect to the lingual on full thickness. You will separate the buccal periosteum from the flap you've just raised. Place the implants, and suture the lingual flap to the periosteum coronal border, leaving most of it exposed.
